Hayden Panettiere reportedly received wellness check 1 week before her death

One week before Hayden Panettiere’s death, authorities reportedly conducted a wellness check at her West Hollywood home.

The Heroes and Nashville actress was found unresponsive and pronounced dead in an apartment in South Carolina on Aug. 16.

The Greenville County Coroner’s Office determined that Panettiere died from “toxic effects of fentanyl, 4-ANPP, Alprazolam, Methocarbamol, and Quetiapine.”

New details have emerged about the days leading up to Hayden Panettiere’s untimely death at 36.

One week before the Heroes star was found unresponsive and pronounced dead in a South Carolina apartment, law enforcement reportedly performed a wellness check on her Los Angeles area home.

The L.A. County Sheriff’s Department received a call at 10 p.m. on Aug. 9 from someone requesting a welfare check on a woman at a residence in West Hollywood, according to a call report obtained by Us Weekly.

Per the log, the caller was “concerned” that the white female — who was not identified in the heavily redacted call report — was “not answering the phone” and asked authorities to perform a wellness check. The redacted report did not identify Panettiere, but a source is said to have confirmed her identity to Us Weekly.

Entertainment Weekly has reached out to the Sheriff’s Department and representatives for Panettiere for comment.

Panettiere died Aug. 16, five days before her 37th birthday. Her on-and-off boyfriend Brian Hickerson and his brother, Zach, found her unresponsive in an apartment in the city of Greenville. The Greenville County Coroner’s Office said that first responders discovered the actress “in cardiac arrest” after answering the 911 call at approximately 1:51 p.m.

The Greenville Police Department said that a preliminary investigation into the actress’ death had “not indicated any signs of foul play or suspicious circumstances.” Additionally, Panettiere’s Aug. 17 autopsy revealed “no signs of trauma” that “would have contributed to the death.” Officials discussed an apparent “overdose” while treating the actress, according to 911 audio obtained by PEOPLE.

An official cause of death for the actress was determined last month, with the coroner’s office stating that she died of “toxic effects of fentanyl, 4-ANPP, Alprazolam, Methocarbamol, and Quetiapine.” The manner of death was ruled as an accident.

In the wake of those findings, publicist Kasey Kitchen, who represented the late Nashville star, said in a statement to CNN, “The investigation remains ongoing, and the circumstances surrounding the case are complex. We must all continue to remain patient and allow the investigative process to unfold.”

Panettiere is survived by her 11-year-old daughter, Kaya, whom she shares with ex-fiancé Wladimir Klitschko.
