Holly Madison Explains Her Decision to Get Plastic Surgery After Pregnancy Weight Gain: ‘Just Nothing I Could Do’

Holly Madison revealed she had a lower facelift after struggling with post-pregnancy weight changes

She researched surgeons online and chose a conservative approach to avoid overdone results

Madison has been open about her cosmetic procedures, including breast work, a nose job and hand rejuvenation

Holly Madison is opening up about her cosmetic surgery decisions.

"I've had a lower facelift on my jawline," the former Playboy Bunny turned author and entrepreneur, 46, revealed to host Dr. Sheila Farhang on the Wednesday, June 24, episode of DearMedia's Derm Approved podcast. "I'm really happy I got it done."

"I just felt like after having kids, you know, I gained a lot of weight from my second pregnancy and there was just nothing I could do,” she explained. “There were no lasers or anything that could really do what I wanted it to do so I found somebody whose before and afters I really loved.”

Madison, who also said that she got "a little bit of lipo" under her chin, shares two kids with her ex-husband, Pasquale Rotella. They welcomed their daughter, Rainbow, in March 2013 and their son, Forest, in August 2016.

Addressing jowls and wanting to tighten her lower face were the reasons she wanted to get the procedure done after giving birth. The scars from her surgery, she said, are near her ears but she's not trying to hide them.

"If you look close, you can totally see it, but I don't care,” the former Girls Next Door star told Dr. Farhang of her scars. "I'm kind of open about it. I already talked about it on my podcast."

Of her recovery process, Madison said she was "a little swollen" and took around two weeks off from filming her podcast. She said she didn't want to tell anyone before she got the surgery because she was "superstitious" about recovering.

Madison said she did research online to find her preferred surgeon, sharing that she looked at before-and-after photos when making a decision. She said that she "didn't want to go too far" with the cosmetic work, noting that she takes a "conservative" approach when she goes in for surgery.

"I'd rather have it not go far enough and then I can go back and maybe do more rather than have it go too far and I'm looking like the Joker," she said on the podcast.

Madison was proud to reveal the beauty treatments she continues to do, and also opened up about her breast work, nose job and experimenting with hand rejuvenation.

Being transparent about her beauty procedures isn't anything new for Madison, who previously opened up to PEOPLE in November 2023 about her relationship with it since becoming a mom of two children.

"I'm very relieved because my daughter and my son both roast me for dyeing my hair, they roast me for having extensions," she admitted. "They roast me when I wear a lot of makeup, so I think I'm good."

"Knock on wood, I think they're not going to fall into the trap of trying to make themselves look over-processed or trying too hard for the beauty standard because they are over it," she continued at the time.
